About Us:

Co.As.It. provides government subsidised support to the aged and younger people with a disability in their homes. Our aim is to maximise our clients’ independence, optimise their quality of life, and foster wellbeing to support them to remain at home for as long as possible. Whilst specialising in the CALD (culturally and linguistically diverse background) community, Co.As.It also caters to the general community.

About the Role:

  • Conduct assessments to identify clients needs and goals

  • Develop treatment plans that promote independence and participation in daily activities

  • Recommend home-modifications, equipment and/or other assistive technology to support independence safety and well-being

  • Support clients with trailing equipment

  • Maintain accurate and current client records

  • Work within a multi-disciplinary team to support holistic client care

  • Provide education and training to ensure equipment is fit-for-purpose and used safely

  • Maintain accurate and timely records, including reports that document clinical reasoning for recommendations

About You

  • Recognised degree in Occupational Therapy and current AHPRA registration (no restrictions or conditions)

  • Minimum 2 years’ experience working as in the relevant allied health profession

  • Experience within Aged Care/NDIS and/or Community Care preferred but not essential

  • Excellent time management skills and ability to maintain timely and accurate clinical documentation

  • Excellent communication and interpersonal skills with ability to collaborate with clients, families and with other team members

  • Drivers licence

  • Police Check issued by the Australian Federal Police

  • NDIS Worker Screening Check
