About your opportunity:
The Clinical Nurse (Diabetes Educator) is required to deliver assessment, education and self-management programs at an advanced level to individuals with diabetes and their families or carers, in both an outpatient and inpatient setting at Mareeba Hospital.
About your role:
* Provide support and leadership in the provision of Diabetes Education and care in line with evidence based practice, Chronic Disease Strategy, Australian Diabetes in Pregnancy, Australian Diabetes Society and Australian Diabetes Educators Association Standards.
* Assist Health Workers, nursing staff and medical staff in assessments and referrals of diabetes clients.
* Work with the multidisciplinary team including Dietician, Doctors, Podiatrists and other allied Health members in case management of diabetes clients and participate in team and case management meetings on a regular basis.
* Maintain data, collate and provide reports, utilising IT systems to ensure data information is available for reporting timeframes.
* Effectively communicate at all levels across the organisation to promote effective team functioning.
