Job responsibilities
· Deliver professional, person-centered care and positive behavior support across all shifts.
· Ensure integrated service delivery and follow up, coordinating and communicating as required with other relevant internal, visiting and external providers.
· Support clients with school work, skills development, transport, community activities, social support, recreational activities, daily cooking and cleaning tasks.
· Provide high quality support according to NDIS and company standards to our clients and ensure that they achieve their full potential.
· Understand and acknowledge the NDIS and company duty of care and duty of confidentiality to ensure that all support and information is confidential.
· Undertake relevant administration duties and provide the company with accurate reports such as incident reports, medication charts, injury reports, communication book notes, case notes etc. Ensure that all paperwork and information is recorded accurately and in a timely manner according to company standards.
· Work within a legal and ethical framework.
· Be proactive in-service delivery that promotes recovery and focus on client strengths.
· Ensure that you support the client with daily living (shopping, cooking, cleaning, hygiene etc.) at the agreed level of participation based on individual needs.
· Work professionally and sensitively with the client and their family, community and support network.
· Support clients and their best interest to make choices about their present and future in regards to education, health, careers, medication and other social situations.
· Organize and participate in appointments and activities with clients by working directly with them and their personal issues and needs.
· Listen to clients, help them establish goals, mentor them and always support them in their best interest.
· Organize community-based activities for clients and liaise with team leaders to provide the best care for clients.
· Ensure that the client’s house is well maintained according to work health and safety standards. Ensure that the client’s house is cleaned and tidy.
· Follow all reasonable directions from senior staff and model a strong work ethic.
· Be ‘hands on’ and involved in direct service delivery.
· Attend all appropriate trainings and house meetings that are relevant to our client’s care.
· Attend to all shifts in a timely manner.
Qualities of a successful applicant
· Flexible with availability to work weekends, weekdays, mornings, afternoons and overnight (both active and passive) shifts.
· Ability and experience working independently on a 1 on 1 basis with disability clients.
· Experience working with clients who have complex mental health needs.
· Willingness to work flexible hours is a MUST
· Punctual, honest and hard working
· Strong written and verbal communication skills, excellent interpersonal skills and a strong ability to solve problems and de-escalate crisis situations
· Must be fully vaccinated against COVID-19 as per SA Health guidelines for the disability sector
· Enrolled Nurse background (HIGHLY DESIRED)
· Exceptional knowledge in medication management, dietary management. (HIGHLY DESIRED)
· Experience in manual handling, mobility assistance and personal care. (HIGHLY DESIRED)
Qualifications and Licenses
· Certificate 4 in disability and Mental health or diploma or a relevant field (essential).
· First aid, mental health first aid, MAPA and medication training certificates (essential).
· Have a current and valid unrestricted driver’s license and reliable access to a vehicle. (essential)
· Have a current DCSI, working with children, vulnerable people and NDIS check.
· Have a current and valid police clearance.
