Description:

We are looking for Community Support Workers who have experience working in an outreach capacity with disability, mental health, personal care, and access to the community.

If you have previously worked in the disability sector, with clients with complex needs, or mental health outreach programs, we want to hear from you. People who have only worked in aged care, or have always worked in home based or facility type settings need not apply.

Successful applicants will have a history of developing long term relationships with clients and thrive on the knowledge that they are making a difference in someone’s life, have relevant qualifications, be available on weekends, have an understanding of the strength-based approach, and have recent experience working in an outreach position independently and as part of a team.

Essential Criteria:

  • Certificate III or higher in Disability, Mental Health or Community Services

  • NDIS Workers Screening Check; First Aid Certificate with up-to-date CPR

  • Current Victorian Drivers’ License, with a registered and reliable vehicle (with Comprehensive Insurance)

  • Infection Control Certificate (COVID)

  • NDIS Worker Orientation Module

  • Mobile phone and access to the internet

  • Previous experience as a support worker in the community.

  • Understanding of the client-centered model and the strength-based approach

  • Proven track record of performing duties in a manner that promotes the independence and dignity of clients to meet their individual goals

  • Highly developed communication and interpersonal skills

  • Ability to manage professional relationships with multiple stakeholders including participants, their families, inter-agency staff, community agencies, and other team members

  • A demonstrated ability to work both independently and as part of a team

  • Accurate record-keeping, time management, and accountability

  • Ability and willingness to identify opportunities for improvements in risk management, service delivery and program development

This position is casual, and remuneration is based on the SCHADS Award 2020 based on qualifications, skills and experience.
