Support Worker - Youth, Disability, Aged - Rockhampton & Gladstone

Currently we are building our workforce pools across the Rockhampton & Gladstone area so we can provide support to our ever-growing client base in the region. Duties include:
- Build and maintain positive relationships with the people you support.
- Follow established care plans.
- Collaborate with relevant stakeholders.
- Respond to crisis situations, offering immediate support and collaborating with appropriate authorities.
- Maintain accurate records of interactions, assessments, and program outcomes.
- Work positively and collaborate with team members to ensure the highest care and support provided.
Requirements:
YOUTH
- Cert IV in Child, Youth and Family Intervention or other relevant field (eg: Disability, Mental Health etc.) - Or working towards
- Hope & Healing Certificate (or willing to obtain)
DISABILITY
- Cert III in Individual support or another relevant field.
AGED
- Cert III in Individual support in Ageing or Cert III in Health Assistance
- Pursuing Diploma in Nursing (2nd year and completed one placement)
- Pursuing Bachelor of Nursing (2nd year and completed one placement)
ALL
- Current First Aid and CPR (or willing to obtain)
- Current NDIS Worker Screening (or willing to obtain)
- Current (Paid) Working with Children check (or willing to obtain)
- Recent and relevant experience in a similar role
- 2 professional references
- Unrestricted Australian Driver's License (required if providing community access support, driving facility vehicles)
- Comprehensive Car Insurance (or willing to obtain - required if using own vehicle for work purposes)
Driver's license is Required.
