About the Role

We are seeking passionate and experienced professionals to join our team as Clinical Assessors (ACAS) across a multiclass role (RN2/AHP2), with classification based on qualifications and experience. In this dynamic position, you will undertake comprehensive clinical assessments of aged clients in community and hospital settings. Using the Integrated Assessment Tool (IAT) and the My Aged Care Assessor portal, you will capture accurate information to identify each client's needs and connect them to the most appropriate care pathways—whether that be in-home support, residential care, or respite services.

You'll collaborate with clients, carers, health professionals, and service providers to ensure every person assessed receives holistic and informed guidance aligned with both State and Commonwealth funding streams under the Aged Care Act 1997. You'll play an important role in meeting assessment benchmarks and KPIs, supported by the My Aged Care Assessment Manual and national guidelines.   • Skilled & Experienced: Extensive experience in aged and community care assessments, including working with clients with complex needs such as frailty, dementia, disability, or terminal illness. You’re familiar with sector reforms and knowledgeable in aged care legislation and pathways.

  • Technically Proficient: Confident navigating My Aged Care systems, including myAssessor and client databases. You’re adaptive to changing technologies and contribute meaningfully to agency development and service innovation.

  • Leadership & Strategic Thinking: Proven ability to work autonomously and under pressure, linking strategic goals with operational practices. You’re a natural leader—building networks, driving performance, and fostering intersectoral collaboration.
