Aged Care On-site Pharmacists
Optimal Medication Solutions provides Medication Management Services to Aged Care Facilities and community clients within Western Australia.
We are a Pharmacy with a difference, whilst we are a PBS Approved Pharmacy, we don’t have the distractions of a regular retail pharmacy. Our single-minded value offer is welcomed by the facilities and residents we serve alike and as a result we keep growing and we need to add more talented people to our team.
Our embedded Pharmacists have the ongoing support and access to the various department team leaders . Our aim is to ensure the best outcome in every instance and that our embedded Pharmacists feel they have the resources to perform their job to the highest level of service to our facilities.

We are looking for talented people with the passion to go the extra distance and provide superior care and problem solving to the Residential Aged Care Facilities (RACF) we service.
We are determined that Optimal Medication Solutions (OMS) can be a proactive partner with the facilities and their team to be much more than just a packing supply pharmacy and become a key resource to help them deal with the challenges they face.
Your role will be many and varied so you will need to develop a deep understanding of our business and also the needs of our facilities and take ownership to resolve any concerns our facilities and residents may have.
Being proficient and fluent in communication is an absolute necessity to build strong, collaborative relationships with our facilities, residents, and prescribers.
Your key responsibilities will include:
Aged Care On-site Pharmacist (ACOP)

Work onsite at a RACF providing direct support on behalf of OMS.

Conduct onsite medication reviews to provide evidence-based recommendations.

Educate and support aged care staff and residents in medication management.

Collaborate with doctors, nurses, and allied health professionals to deliver tailored healthcare.

Promote safe and effective use of medicines and implement strategies to enhance medication safety.

Liaise with residents and their families by providing education about medication use, safety, and potential side effects.

Attendance at MAC meetings and provision and support of required reporting and QUM activities.

Medication management audits including maintenance of trolleys, psychotropic drug use.

Provision of clinics and health promotion activities i.e. vaccination services, clinical newsletters.
About You
To succeed in this role, you will:

Be a registered pharmacist with AHPRA.

Have current professional indemnity insurance.

Be qualified to provide vaccination services.

Have completed or are currently studying MMR + ACOP training credentials.

Have experience in aged care, community pharmacy, or hospital pharmacy.

Have excellent communication and interpersonal skills and the ability to work collaboratively with other teams.
